About the PACT Grant
Fertile Action’s Parenthood After Cancer Treatment (PACT) Grant was established to provide financial support for female cancer survivors and provide them with access to third party reproductive services they otherwise would not be able to afford. The Grant is NOT a cash award but is aimed at  significantly reducing the cost of fertility preservation, sperm donation, egg donation, surrogacy and long-term storage for cancer survivors through service providers (California Cryobank, Reproductive Endocrinologists, Egg and Surrogacy Agencies, Psychologists and Reproductive Attorney’s) which we have partnered with to reduce their fees or donate their services to the grantee. The Grant does not include egg donor or surrogate fees, which will be the sole responsibility of the grantee.

Fertility Preservation grants are 100% accepted and based solely on having cancer. If you reside in California, we generally can navigate you to a service provider within 24 hours. If you reside outside of California, we may need 48 - 72 hours to line you up with someone. If you already started with a provider not already in our network, we may be unable to help. Email right away even after applying so we can help immediately. 

Each applicant must select which PACT Grant to apply for. We offer three types of PACT Grants with one award to a grantee per category:

  • Fertility Preservation (egg or embryo cryopreservation)
  • Sperm donors
  • Long-term storage
  • Egg Donor Grant – Includes a reduced-fee egg donor cycle.  It may not include medications, labs or ultrasounds unless stated otherwise.
  • Surrogacy Grant – Includes a reduced-fee surrogacy cycle.  It may not include medications, labs or ultrasounds unless stated otherwise.

What the PACT Grant Does NOT Cover
The PACT Grant may not cover IVF medications, labs, ultrasounds, egg donor fees or surrogacy fees.  There also may be other additional costs incurred that will be the responsibility of the grantee.  Fertile Action is not financially responsible for any costs incurred beyond the specific services provided for under the PACT Grant award.  The grantee may be required to travel out of state to accept the terms of the Grant.
The PACT Grant is a bi-annual grant with application submission deadlines occurring on a rolling basis.

Fertile Action does not guarantee any of the work performed by the Service Providers or successful results.

All information submitted to Fertile Action will be held in strictest confidence and viewed only by the selection committee. The $50 application fee is a non-refundable PACT donation and no materials submitted by grantees (application, photos, letters, etc.) will be returned. Fertile Action will not be responsible for the inaccuracy of any medical clearances provided by grantees’ medical providers as part of the application process. You must be at least 18 years old to apply.

Application Components and Process
The application process is lengthy based on which grant you are applying for, but you may save your work and return to finish it at any time. In some, but NOT ALL cases, you will need to submit the following:

  1. Personal Information Form
  2. Application Information 
  3. Income Verification - Last Two Years of Tax Returns
  4. Income Verification - Pay Stubs or Contract 
  5. Driver's License Copy
  6. Health Insurance Card Copy 
  7. Personal Essay (Uploaded) 
  8. Letter to Your Future Child (Uploaded)
  9. Letter of Medical Necessity from Your Reproductive Endocrinologist 
  10. Medical Clearance Letter from Your Oncologist
  11. Two Personal Letters of Recommendation 

Because our funds and service donations are limited, we are not able to fund all worthy applications received.  Unfortunately, some applications may be declined regardless of their individual merit. All fertility preservation applications are approved. A selection committee will review the other applications and score the different components as objectively as possible. You will be notified when the grantee is selected.
